Join PatientVoices

All registered patients over 16 are welcome to join the PatientVoices group.

Meetings are held monthly and any PV member is welcome to attend. However, if you do not wish, or are unable, to attend we will still keep you informed of our activities by regular mailings with information of interest to patients. This will include Minutes of our meetings.

PatientVoices Registration

By submitting this form and joining PatientVoices, I confirm that I am over 16 and a registered patient with Peartree Group Practice. I understand that:

1. I will receive an e-mail from PatientVoices to confirm receipt of this application

2. I will receive occasional mailings of interest and relevance to PatientVoices in particular and the NHS in general, including Minutes of PatientVoices meetings

3. My details, as above, will only be used for the above purpose and will not be divulged to any third party

4. I am entitled to attend any PatientVoices meeting, usually held on the second Thursday of every month at the Peartree Surgery. The times of the meetings will be published on the Agenda.

5. As a member I am entitled to vote on any matter within PatientVoices business including the Annual General Meeting.

6. I may unsubscribe from the mailinglist at any stage
